Ukraine launched a long-awaited counteroffensive in the Russian-occupied Kherson region Monday, breaking through Russia’s front line in the southeastern Ukraine oblast, its military said in a statement Monday.

A year after U.S. forces killed seven children and three adults in an airstrike in Afghanistan, the Pentagon unveiled an action plan Thursday aimed at reducing civilian casualties during American military operations.
